5. Payment Terms

Payment Authorization

Payment Plans

Late Payments and Fees

Refunds

Refund requests must be directed to your insurance provider. We process refunds as instructed by the insurance company. Refunds typically take 7-10 business days to process.

6. AutoPay Terms

If you enroll in AutoPay:

• You authorize automatic charges to your payment method on each billing date
• You must maintain sufficient funds or credit available
• You can cancel AutoPay at any time by contacting us at least 5 business days before the next billing date
• We will notify you if your payment method fails and provide alternative payment options
• You will receive payment confirmations and reminders via email or SMS

7. User Responsibilities

You agree to:

• Use our services only for lawful purposes
• Provide accurate and truthful information
• Comply with all applicable laws and regulations
• Not attempt to interfere with or disrupt our services
• Not use our services for fraudulent activities
• Not attempt unauthorized access to our systems or data

8. Intellectual Property

All content, features, and functionality of our services, including but not limited to text, graphics, logos, images, and software, are owned by US Policy Billing and are protected by copyright, trademark, and other intellectual property laws. You may not copy, modify, distribute, or reproduce any content without our express written permission.

9. Limitation of Liability

To the fullest extent permitted by law, US Policy Billing shall not be liable for:

• Any indirect, incidental, special, consequential, or punitive damages
• Loss of profits, revenue, data, or use
• Damages resulting from your use or inability to use our services

• Unauthorized access to or alteration of your transmissions or data

Our total liability shall not exceed the amount you paid to us in the 12 months preceding the claim.

14. Dispute Resolution

Governing Law

Arbitration
