3. Eligibility

4. Account Registration and Security

- Maintain and promptly update your account information

5. Payment Terms
Payment Authorization
By providing payment information, you authorize us to charge your designated payment method for all amounts due according to your selected payment plan.
Payment Methods
We accept various payment methods including credit cards, debit cards, bank transfers, and electronic payments. All payments are processed securely through encrypted channels.
Payment Plans
We offer flexible payment plans (monthly, quarterly, annual). You may change your payment plan subject to approval and any applicable fees. Changes must be requested at least 15 days before your next billing date.
Late Payments and Fees
Payments not received by the due date may be subject to late fees as specified in your insurance policy. Failure to make timely payments may result in policy cancellation or lapse of coverage as determined by your insurance provider.
Refunds
Refund requests must be directed to your insurance provider. We process refunds as instructed by the insurance company. Refunds typically take 7-10 business days to process.
6. AutoPay Terms
If you enroll in AutoPay:
- You authorize automatic charges to your payment method on each billing date
- You must maintain sufficient funds or credit available
- You can cancel AutoPay at any time by contacting us at least 5 business days before the next billing date
- We will notify you if your payment method fails and provide alternative payment options
- You will receive payment confirmations and reminders via email or SMS
7. User Responsibilities
You agree to:
- Use our services only for lawful purposes
- Provide accurate and truthful information
- Comply with all applicable laws and regulations
- Not attempt to interfere with or disrupt our services
- Not use our services for fraudulent activities
- Not attempt unauthorized access to our systems or data
8. Intellectual Property
All content, features, and functionality of our services, including but not limited to text, graphics, logos, images, and software, are owned by US Policy Billing and are protected by copyright, trademark, and other intellectual property laws. You may not copy, modify, distribute, or reproduce any content without our express written permission.
9. Limitation of Liability
To the fullest extent permitted by law, US Policy Billing shall not be liable for:
- Any indirect, incidental, special, consequential, or punitive damages
- Loss of profits, revenue, data, or use
- Damages resulting from your use or inability to use our services
- Errors or inaccuracies in content or information
- Unauthorized access to or alteration of your transmissions or data
Our total liability shall not exceed the amount you paid to us in the 12 months preceding the claim.
10. Indemnification
You agree to indemnify, defend, and hold harmless US Policy Billing, its officers, directors, employees, and agents from any claims, liabilities, damages, losses, and expenses arising from your use of our services, violation of these terms, or infringement of any rights of another party.
11. Third-Party Services
Our services may contain links to third-party websites or services. We are not responsible for the content, privacy policies, or practices of third-party sites. Your use of third-party services is at your own risk.
12. Termination
We reserve the right to suspend or terminate your access to our services at any time, with or without notice, for any reason, including violation of these terms. Upon termination, you remain responsible for all outstanding payments and obligations.
13. Modifications to Terms
We may modify these Terms and Conditions at any time. We will notify you of material changes by posting the updated terms on our website and updating the "Last Updated" date. Your continued use of our services after changes are posted constitutes acceptance of the modified terms.
14. Dispute Resolution
Governing Law
These Terms and Conditions shall be governed by and construed in accordance with the laws of the United States and the state in which our principal office is located, without regard to conflict of law principles.
Arbitration
Any dispute arising from these terms shall be resolved through binding arbitration in accordance with the American Arbitration Association rules. You waive your right to participate in class action lawsuits or class-wide arbitration.
